MasterCard, Visa, Discover, and Debit. Personal checks are welcome. As a courtesy to our patients, our office will gladly assist you with the filing of your PPO dental insurance claims. Patients and account guarantors who use dental insurance through our office are required to provide the necessary information needed by our office staff to successfully submit your claims. Patients and account guarantors are responsible for understanding their individual dental insurance policy and benefits.
Payment plan arrangements can be made for patients who do not have dental insurance coverage, and for patients with treatment cases that require special financial planning.
Dental insurance policy information, payment and/or payment arrangements are expected to be made when services are rendered.

What Our Office Requires
You will be asked to complete and sign a patient health and registration form, and insurance form at the office. This is the type of information you will be asked to provide:
Subscribers: name and address, social security number, date of birth, employers name and address.
Insurance Company: name and address, 800 telephone number, group number or policy number.
Patient name and address, social security number, date of birth, group number, employer name and address.
Approximate date of last dental cleaning and last set of dental x-rays. If you are able to request and receive your last set or current dental x-rays prior to your first visit to our office, it will provide useful information during your examination with one of our doctors. (Bite-wing x-rays, panoramic x-ray, or complete full mouth set are what you would ask for.)
